Out of network: CityCare Direct. Out-of-network visits are billed directly, with an itemized statement provided for your insurer.
Payment options
- Pay a bill online
Pay an outstanding balance by card or bank transfer. You will need the invoice number from your statement.
- Set up a payment plan
Split an eligible balance into monthly installments. Plans are available for balances over $250 with terms up to twelve months.
- Get a cost estimate
Request an estimate before a scheduled procedure. Estimates include facility, professional, and imaging fees where applicable.
- Financial assistance
Applications are available for patients with incomes up to 400% of the federal poverty level. Eligible balances may be reduced or waived.
- HSA and FSA cards
Health Savings Account and Flexible Spending Account cards are accepted for eligible services at all locations.
- Out-of-network billing
If your plan is not in network, our billing team provides an itemized statement you can submit to your insurer for reimbursement.
